Baptismal Requirements:


Parishioner (Those who are living within UP Diliman Campus)

  1. Photocopy of Birth Certificate and Marriage Contract (Certified True Copy from the Civil Registrar or National Statistics Office (NSO))

Non Parishioner (Those who are living outside UP Diliman Campus)

  1. Permit from your local Parish

First Communion Requirements:


  1. Sunday School – All interested First Communicants are required to attend one (1) whole school year of our Sunday School Catechism. The Sunday School starts every first week of August until March. The class starts at 10:00 in the morning, and ends at noon. Opening of Sunday School will be announced 3-4 weeks before the start of class. Please remain updated regarding this program through our Facebook Page, Bulletin Board, and after-Mass announcements.
  2. Baptismal Certificate – All First Communicants are required to submit a baptismal certificate that was issued within three months before the First Communion Mass.
  3. Confession

Confirmation Requirements:


Scheduled Confirmation Rites

  • Seminar – Those who wish to be confirmed must first register to the Catechism Seminar for Confirmation every April. There will be an announcement 3-4 weeks before the day of seminar. The Confirmation Rite is usually held every May. Please remain updated regarding this program through our Facebook Page, Bulletin Board, and after-Mass announcements.
  • Baptismal Certificate – All Confirmants are required to submit a baptismal certificate that was issued within three months prior.

Note: Confirmation Rite is every May, after all the scheduled seminars, practice and confession.

 

Holy Matrimony Requirements:


Required Documents to be submitted:

From the Church:

  • Baptismal and Confirmation Certificate – Original copy; with notation (“For Marriage Purpose”); issued within three (3) months before the wedding date. (Required for both parties)
  • Banns – Reply form, duly accomplished and signed by the Pastors of respective parties.
  • Permission of proper pastor – The bride’s Pastor shall issue a permission allowing marriage to be solemnized outside the bride’s parish.
  • 2×2 Picture of Bride and Groom – 2 sets for filling and posting in the parish bulletin.

From the Local Civil Registrar and NSO:

  • Authenticated or certified true copy of birth certificate
  • Certificate of non-marriage (Issued by NSO)
  • Marriage licence (Issued by the Local Civil Registrar)
  • Mariage contract (Certified True Copy, if civilly married)
  • Death certificate (for widows/widowers, an authenticated death certificate of their deceased spouse)

Note: All required documents must be submitted to the Parish Office one week before the day of the canonical interview.

Interviews and Formation Required:

  • Canonical Interview – a Church required process to determine capacity and disposition for marriage and married life. Schedule will be announced.
  • Pre-Marriage Seminar – a seminar conducted by marriage counselors to prepare candidates for the proper disposition in marriage and family life on Sundays, 9:00 AM to 3:00PM.
  • Confession

Schedule of Fees

  • Reservation Fee (P 5,000.00) to reserve the date and time for the wedding.
  • Donation for the use of the church(Any amount) given on or before the wedding day.

**Please bring your own choir if the wedding ceremony falls on a weekday.

Reminders:

  1. Document and Formations are equally important prerequisites of the Sacrament of Matrimony
  2. A reservation fee of P5,000.00 is required upon registration.
  3. Suppliers are required to coordinate with the parish office for accreditation and orientation at least 2 weeks before the scheduled wedding.
  4. Sing-along equipments are not allowed in the church. Invited singer/choir must coordinate with the parish accompanist before the wedding Mass.
  5. In house florist and wedding singers are available upon request. (Separate stipends and fees)
